DOT exams are offered by appointment only on designated clinic days in Monroe, CT.

Who We See:

· CDL drivers

· Non-CDL commercial drivers

· Delivery drivers (Amazon DSP, FedEx, couriers)

· Construction, utility, trade drivers

· Bus drivers

· Owner-operators

What to Bring With You:

Please arrive 10-15 minutes early with:

· Government-issued Driver’s license

· glasses or contact-lenses (if used for driving)

· Hearing aids (if used)

· List of current medications

· Any employer-required DOT paperwork

· Additional completed forms by specialists if needed (MSCA 8570 or 8571)

· Completed Medical history form, MCSA 5875

** Arriving without required items may delay or prevent certification **
